Jake passed away peacefully at the Women’s Hospital at the Health Sciences Centre on Tuesday, June 14, 2011.
Jake is survived by his mother Alice and father Roland. He leaves behind 2 brothers Chase (11), Gage (9) and one sister Chelsea (6). He also leaves behind 2 grandmothers Valere and Joan and one great-grandmother Mary as well as his grandfathers Roland and John and numerous aunties, uncles and cousins.
Jake was predeceased by his grandfather Dennis, Aunties Barbara, Brenda, Fernande and Cheryl and uncles Laverne and Bryan Sr.
A Traditional Service will be held at the Swan Lake First Nation Hall on Saturday, June 18, 2011 commencing at 10:00 a.m. with the Funeral Services at 2:00 p.m. Interment to follow at the Indian Springs Cemetery.
